A telehealth platform's clinicians were spending excessive hours on after-visit documentation - SOAP notes, assessments, treatment plans - instead of patient care.

Constraint

Notes had to land in existing EHR templates. Clinical accuracy was non-negotiable. Unreviewed model text could not auto-commit to the chart. Real-time drafting during visits was required.
